The proof of life certifies the survival of those who receive a pension or retirement check from the Social Security Fund of Panama.

These certifications are made every 6 months or annually.

You may apply for the proof of life at our offices in person at no cost.

Required Documents:

• Proof of life form fully signed in presence of the Consul and properly completed. If you are not able to come to our office, you must sign before a Public Notary.

• Photocopy of your Social Security ID from Panama.

• Photocopy of your Passport or Panamanian ID. If your Panamanian identification is expired, please provide an additional valid ID.

• Self-addressed stamped envelope to return the document to you or pay $40 for FedEx Next Day Air.
