Proof of Life

The proof of life certifies the survival of those who receive a pension or retirement check from the Social Security Fund of Panama.

These certifications are made every 6 months or annually.

You may apply for the proof of life at no cost.

Requirements:

• Proof of Life form, fully completed & signed in presence of the Consul or a Public Notary.

• Photo of your Social Security card from Panama.

• Photo of your Passport, Panamanian Cédula card, or U.S. Driver's License.

• Self-addressed stamped envelope to return the document to you or send $40 for FedEx Next Day Air.
